Academic Petition – In consultation with the faculty advisor or major department chair, submit a petition regarding an academic issue (e.g. medical withdrawal, request a schedule change after the drop/add deadline, enroll in excessive hours).

Declaration of Major/Minor – Officially change or declare a major and/or minor while at FHU.

Intent to Graduate – At the beginning of the semester, students who anticipate graduating should complete this form prior to the graduation application deadline. This form will be used to notify the student’s advisor(s) and the registrar of their intent to complete degree requirements.

Course Substitution Form – Obtain approval for a course (taken either at FHU or another institution) to substitute for a different Liberal Arts course that is required for your graduation.

Course Withdrawal Form  – Submit this form to request withdrawal from an undergraduate level class after the Drop/Add period ends. Consult the university catalog for deadline information.

Professional Licensure Attestation – Students who reside in a state where a degree may lead to professional licensure but FHU’s curriculum may not satisfy all the student’s home state criteria will need to attest that they plan to seek licensure in a state that does align with FHU’s curriculum. Please reference the Accreditation and State Authorization and Professional Licensure Disclosure webpages to obtain further information, including contact information for the applicable licensing board. After reviewing the provided information and considering your future plans, you may affirm your intention to seek licensure in one of the states that accept FHU’s curriculum by completing the Professional Licensure Attestation form.

Field Lab SU24 – 1–9 hours credit each. Total maximum 9 hours. Field laboratory is a carefully planned activity based on college level prerequisites or corequisites. Specific objectives, activities, and evaluative criteria are designed and effected jointly by a faculty member and the field supervisor. A field laboratory includes either private or classroom instruction by a faculty member, who also determines the final grade.

Practicum or Internship – 1–9 hours credit. A practicum or internship is a professional field experience under the supervision of an experienced professional carefully selected and approved by the University. Close contact is maintained between the field supervisor and the faculty supervisor.

Independent Study (Undergrad)Independent Study (Grad) – 1–4 hours credit each. An independent study is a reading-research course or special project planned in consultation with and guided and evaluated by an instructor. Open to students with an institutional GPA of 2.50 or above. Grading will be “pass/fail” or conventional at the discretion of the instructor. It may not duplicate a course listed in this catalog

Graduate Studies Change of Program or Advisor – Request a change of program or advisor for your graduate studies at FHU.

FERPA Privacy Release – Your non-directory information will not be released without your consent. In order for FHU to discuss your bill, academic progress, or grades with anyone else, including members of your immediate family, you must provide permission in this form.

Transient Course Request – Request approval to take a course at another institution to satisfy a requirement for your degree at FHU.

IRB Amendment, Extension, Appeal, or Notice of Completion – Request approval to make an amendment or continue an approved IRB project, appeal an IRB decision about a requested project, or notify the IRB of your project completion.

IRB Not Human Subjects Research – Request a determination from the Freed-Hardeman University IRB whether or not your research activities are Human Subjects Research, as defined by federal regulations.
